Homecare Outreach and Sales Manager
We are an established Medicaid and Managed Care Home Care Agency, with an Adult Foster Care Program(AFC) and Group Adult Foster Care Program (GAFC). In our programs family members and friends are compensated to provide services to their loved ones at the comfort of their homes. We service medicaid members aged 22 years and above who meet eligibility criteria. We are seeking a highly motivated and experienced Regional Outreach and Sales Manager to join our team. As a Regional Sales Manager, you will be responsible for overseeing and driving outreach, marketing and sales activities within your assigned region (Western Mass and Worcester County) Your primary goal will be to meet or exceed our admissions quota, expand our customer base, and maintain strong customer relationships and community partnerships.
Duties
– Develop and implement outreach strategies to promote company Adult Foster Care (AFC) services
– Build and maintain relationships with potential clients through effective communication and networking
– Conduct market research and analysis to identify new business opportunities
– Collaborate with the sales team to develop sales strategies and achieve revenue targets
– Utilize Salesforce or other CRM tools to track leads, manage accounts, and monitor sales activities
– Provide exceptional customer service by addressing client inquiries and resolving issues in a timely manner
– Stay up-to-date with industry trends and competitor activities to identify areas for improvement
Requirements
– Proven experience in Outreach, sales, and/or business development
– Strong background in Homecare/ Home Health outreach and sales
– Bilingual fluency in English and Spanish.
– Excellent communication and interpersonal skills
– Ability to analyze market trends and identify potential opportunities
– Proficiency in using Salesforce or similar CRM software
– Self-motivated with a strong drive for results
– Ability to work independently as well as collaboratively within a team environment

HR Coordinator

Duties:
– Serve as the primary point of contact for Employees, providing exceptional customer service and addressing their inquiries and concerns
– Schedule appointments and manage the calendar for the office
– Maintain accurate client records and update information as necessary
– Coordinate with healthcare providers and insurance companies to ensure smooth communication and billing processes
– Assist with medical administrative support tasks such as filing, data entry, and organizing documents
– Collaborate with the healthcare team to ensure efficient workflow and patient care coordination
Qualifications:
– Previous experience working in an office or dental/medical office setting is preferred
– Familiarity with medical terminology and procedures is a plus

Responsibilities
The responsibilities of the RN field nurse in our programs include but not limited to:
- Completes initial home health assessment of patient/family to determine home care needs, completes physical assessment of current and previous illnesses.
- Completes nursing assessments such as neuro, skin, vitals, and overall health assessment, reviews all potential participants in the Group Adult Foster Care Program.
- Conducts home visits as assigned, assessing the patient’s condition and progress toward goals.
- Coordinates the implementation of physician’s orders with client and Program Personnel.
- Conducts Oasis assessments
- Coordinates all contacts with physicians and certified home health agencies and PCA agencies as needed.
- Completes all necessary nursing documentation, including OASIS/MDS/CDS, nursing progress notes after patient assessment.
- Participates with in the planning, implementation and evaluation of participants, service plans, and other program components.
- Report changes in the member’s condition to the member’s physician. Acts as a liaison with area hospitals and different community agencies as needed.
- Provide patient education regarding hygiene, safety, and health concerns.
- Supervises the LPNs/CNA/HHA and or PCAs in the Program.
